Notice of Privacy Practices

Effective Date: 10/05/2026

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our Responsibilities

Ashley Wilson Medical Aesthetics LLC (the Practice) is required by law to maintain the privacy and security of protected health information (PHI), provide you with this Notice of our legal duties and privacy practices, follow the Notice currently in effect, and notify affected individuals following a breach of unsecured PHI when required by law.

How We May Use and Disclose Your Health Information

We may use or disclose your PHI without your written authorization when permitted or required by law, including:

  • Treatment: To provide, coordinate, or manage your care and communicate with other health care providers involved in your treatment.

  • Payment: To bill, collect payment, process transactions, and carry out related payment activities.

  • Health Care Operations: For activities necessary to operate the Practice, such as quality assessment, compliance, business management, and professional review.

  • As Required or Permitted by Law: For certain public health activities, health oversight, mandatory reporting, judicial or administrative proceedings, law enforcement purposes, or to prevent or lessen a serious and imminent threat when applicable law permits or requires the disclosure.

Other uses and disclosures of PHI will be made only with your written authorization when an authorization is required. You may revoke an authorization in writing, except to the extent we have already acted in reliance on it.

Your Rights

Subject to applicable law and limited exceptions, you have the right to:

  • Inspect and obtain a copy of health information maintained about you in the designated record set.

  • Ask us to correct or amend health information you believe is incorrect or incomplete. We may deny a request in certain circumstances and will explain a denial when required.

  • Request restrictions on certain uses or disclosures. We are generally not required to agree, except where HIPAA requires us to honor a qualifying restriction.

  • Request that we communicate with you in a reasonable alternative way or at an alternative location.

  • Request an accounting of certain disclosures of your PHI.

  • Obtain a paper copy of this Notice at any time, even if you agreed to receive it electronically.

  • Choose someone with legal authority to act for you regarding your health information.

  • File a privacy complaint without retaliation.

Your Choices

In certain situations, you may tell us your preferences about sharing information, such as communications with family members or others involved in your care. We will follow your instructions when required by law and otherwise use professional judgment as permitted by law.

Marketing, Sale of PHI, and Other Authorization-Based Uses

We will obtain your written authorization before using or disclosing PHI for purposes for which HIPAA requires authorization, including most uses and disclosures for marketing and any sale of PHI as defined by HIPAA. Separate laws may impose additional requirements on consumer health data.

Confidential Communications

We will accommodate reasonable requests to receive communications about your health information by alternative means or at alternative locations when required by law. Please tell us how you prefer to be contacted.

Changes to This Notice

We may change this Notice and make the revised Notice effective for PHI we already maintain as well as information we receive in the future. The current Notice will be available from the Practice and posted as required.
